Remote consultation

Telehealth abortion care

A telehealth consultation lets you speak to a healthcare professional by phone or video. Your clinician decides, with you, whether remote care is clinically appropriate or whether you need to be seen in person.

At a glance

  • A private consultation by phone or video
  • Held to the same clinical standard as an in-person consultation
  • Not suitable for everyone — your clinician decides with you
  • An in-person visit may still be needed

What telehealth abortion care means

Telehealth (also called telemedicine) means receiving healthcare at a distance — usually a consultation by phone or video. For abortion care, it means your consultation and assessment can begin without travelling to a clinic.

It does not mean care without a clinician. A registered healthcare professional still takes your history, assesses your situation and is responsible for the care you receive.

Who it may suit

Remote consultation can be helpful if you live far from a clinic, cannot easily take time away from work or family, or simply want to ask questions before deciding anything. Whether treatment can also be provided remotely is a separate, clinical question.

The World Health Organization notes that, in the first 12 weeks of pregnancy, a medical abortion can be self-managed in whole or in part outside a health facility, provided the person has accurate information, quality medicines and support from a trained health worker if they want it. Your clinician applies this to your individual circumstances and to South African requirements.

How clinical suitability is decided

Your clinician needs to be confident about a few things before recommending care at a distance, including how far along the pregnancy is, whether there are any warning signs that need investigating, and whether you could reach in-person or emergency care if you needed it.

If any of these are uncertain, you will be advised to attend a clinic. This is not a refusal of care; it is the clinician doing their job properly.

What telehealth cannot do

A phone or video call cannot replace a physical examination or a scan when one is needed. It also cannot confirm your eligibility in advance: we do not use online questionnaires or automated tools to decide who can be treated.

After your consultation

Whatever you decide, you will know how to reach us. If you go ahead with treatment, follow-up and aftercare are part of your care. If remote care is not suitable, we will explain your in-person options, including medical abortion and surgical abortion at a clinic.

When to get medical help

Your clinician will tell you exactly what to expect and what is not normal for your situation. As a general guide, contact a healthcare professional promptly if, after an abortion, you have:

  • very heavy bleeding, or bleeding that is much heavier than you were told to expect
  • a fever or chills that do not settle
  • severe pain that is not eased by the pain relief you were advised to take
  • vaginal discharge that smells unpleasant or unusual
  • dizziness, fainting, or feeling very unwell
  • signs that you may still be pregnant in the weeks afterwards

If it feels like an emergency, do not wait. Call 112 from a mobile phone or 10177 for an ambulance, or go to your nearest emergency unit. For anything else, call us on +27 65 503 9462.

The process

How a telehealth consultation works

This is the general sequence. Your clinician will adapt it to your situation and tell you what to expect at each stage.

  1. Initial enquiry

You contact us by WhatsApp, phone or the enquiry form and tell us you would like a remote consultation.

  2. Clinical consultation

A healthcare professional speaks with you by phone or video at an agreed time, somewhere you feel able to talk privately.

  3. Medical assessment

The clinician asks about your pregnancy, your health and your medical history, in the same way they would in a clinic.

  4. Eligibility assessment

They decide whether what they know is enough to care for you safely at a distance, or whether you need a scan, an examination or an in-person visit.

  5. Treatment discussion

Your options are explained, including the risks, the alternatives and what each would involve. You decide whether and how to proceed.

  6. Clinician-directed care

Any treatment is provided on your clinician's instructions, with clear written guidance and a way to reach us.

  7. Follow-up support

We follow up to check that you are well and that treatment is complete, and arrange in-person care if it is needed.

Frequently asked questions

Questions about telehealth abortion care

Can every patient be treated remotely?

No. Remote care is only appropriate when a clinician is satisfied that they can assess and care for you safely without seeing you in person. Some people will be advised to attend a clinic for a scan, an examination or treatment.

Will I ever need to visit a clinic?

Possibly. Your clinician may ask you to come in before treatment, or afterwards if follow-up suggests you should be seen. We will always tell you why.

Is a telehealth consultation as thorough as one in person?

Healthcare professionals in South Africa are expected to provide telehealth to the same standard of care as an in-person consultation, and they remain responsible for deciding when remote care is not appropriate.

How do I keep a telehealth consultation private?

Choose a time and place where you can speak freely. Tell us how you would prefer to be contacted, and whether it is safe to leave a message. You can ask us to use WhatsApp, a call or email.

What do I need for a remote consultation?

A phone with a reasonable signal or data connection, and a few uninterrupted minutes. It helps to know the first day of your last period and to have a list of any medicines you take.
